The overarching aim is to promote independence, reduce the risk of homelessness, and ensure safe, well‑managed transitions back into the community for adults with complex mental health needs. The appointed provider will deliver daily housing‑related support, key‑working, risk and safety planning, tenancy sustainment, and move‑on support, working collaboratively with mental health teams to maintain continuity of care and effective risk management. This notice is an intention to award a contract under the Most Suitable Provider Process. This a new service with an existing provider. The contract is for a period of 18 months commencing on 01 April 2026 to 30th September 2027.
