    "description": "Nottingham City Council, on behalf of the Nottingham City Place Based Partnership, is seeking tenders from sufficiently experienced and qualified organisations to deliver a range of services to improve outcomes for people who experience severe and multiple disadvantage (SMD). \r\nSMD is defined as concurrent experiences of three or more of: homelessness, substance use, mental health issues, domestic and sexual violence or abuse, and contact with the criminal justice system.\r\nNottingham City Place Based Partnership is committed to improving the outcomes for people experiencing SMD and aims to procure \u0027Changing Futures\u0027 services as part of meeting these needs. Nottingham has secured ongoing funding through the Integrated Care Board\u0027s Health Inequalities Innovation Improvement Fund.\r\n\r\nThe Services will be commissioned in two Lots: \r\nLot 1 is the Main Delivery Service (MDS) \r\nLot 2 is the Partnership Learning Hub (PLH).  \r\nProviders are invited to tender for Lot 1 and/or Lot 2.\r\n\r\nLot 1 - The Main Delivery Service comprises the required Core functions:\r\nFront Line Delivery\r\n\r\na.\tIntensive Case Management \r\nb.\tMulti-Disciplinary Team\r\nc.\tPeer Mentoring\r\nSystem Development\r\nd.\tLived Experience Coordination\r\ne.\tLearning and Evaluation\r\n\r\nLot 2 - The Partnership Learning Hub: training and shared learning to all system partners and services in Nottingham.\r\n\r\nChanging Futures anticipates attracting additional funding over the period of the contract and should additional resources become available, enhanced services may be added to the contract, as explained in the tender documents.  \r\nThe contract duration for both Lots will be for 3 years with a possible extension of 2 years at the Council\u0027s discretion and subject to continued funding from the ICB. \r\nThe total contract value for both Lots is estimated to be a maximum of \u00a311,051,905 over the total length of the contract, including any potential extensions and possible additional funding (this would be a maximum of \u00a39,858,625 for Lot 1 and a maximum of \u00a31,193,280 for Lot 2).\r\n\r\nThe contract start date will be 1 April 2025.\r\n\r\nThis contract will be let under the Light Touch Regime of the Public Contract Regulations 2015.\r\n\r\nApplication documents can be downloaded from www.eastmidstenders.org \r\n\r\nThe closing date for receipt of completed Application Forms is 12 noon, Monday, 8 November 2024",
