These Individuals may have  additional needs in relation to their mental health, physical disabilities or autism.\r\n      \r\n      The service provider will work with Care Managers, the Commissioning Team and Sovereign Network Group (The Registered Social Landlord) to support the individuals and their families in the transition and ongoing support within this service.",
