    "description": "Please note this contract opportunity is closed, and this is an award notice (for the relevant contracts).\r\nThe aim of the Prevention and Early Intervention Services is to support Enfield residents to access the right information and support, at the right time. This means that people are enabled to live independently for longer, with good outcomes and without the need for more intensive support from health or social care. \r\nThe Service is divided into five Lots with each Lot envisaged to have a separate contract. The 5 individual Lots are as follows:\r\nLot 1: Supporting Unpaid Carers\r\nLot 2: Supporting Independence through Information \u0026 Advice Digital Inclusion \u0026 Assistive Technology \r\nLot 3: Supporting Independence through Advocacy \u0026 Inclusion \r\nLot 4: Supporting Independence \u0026 Inclusion Through Early Intervention\r\nLot 5: Home from Hospital, Supporting Recovery \u0026 Maintaining Independence\r\nThe contracts are let for initial term of 3 years.
