    "description": "Medway Council are reprocuring an Intermediate Care and Reablement Service. \nThe service will include home-based intermediate care and reablement for Pathway 1 acute hospital discharges and Bed-based intermediate care and reablement for Pathway 2 acute hospital discharges as well as more complex care such as bariatric and non-weight bearing pathways, as one service.
