This service will provide short-term, supported accommodation for single adults who are currently sleeping rough, with the aim of facilitating rapid assessment, stabilisation, and transition into longer-term housing. Service Objectives \u2022 Provide immediate, safe, and supported accommodation for rough sleepers \u2022 Conduct holistic assessments within 72 hours of entry \u2022 Develop personalised support and resettlement plans \u2022 Facilitate access to health, social care, and housing services \u2022 Support residents to transition into sustainable accommodation Target Group \u2022 Adults aged 18+ who are rough sleeping \u2022 Individuals with complex needs including mental health, substance misuse, or offending history \u2022 Referrals accepted only via the Council\u2019s Rough Sleeping Coordinator through Pathway Planning meetings Accommodation Requirements \u2022 14 self-contained or semi-self-contained units \u2022 Communal space for meals and social interaction \u2022 Office space for staff and confidential meetings \u2022 Secure access and CCTV",
