This contract will operate for three years, plus the possibility of extension by two years subject to satisfactory delivery of the expected outcomes. The service is expected to:\n\u2022 improve the flow of older patients out of hospital\n\u2022 address safety concerns at home and other barriers that prevent timely discharge\n\u2022 ensure that essentials such as food and working utilities (by providing meter top ups, or providing information/referral to services that can resolve any issues) are in place when returning home\n\u2022 improve well-being and confidence by connecting older people discharged from hospital with other services and community resources, and\n\u2022 support or signpost to maximise income\nAll of which will support to reduce the risk of readmission and the need for additional health and social care services further down the line.",
