It is designed for patients considered to be in the last eight weeks of their lives who wish to receive care in their own homes.\u003cbr/\u003e\u003cbr/\u003eLocal Coordination Service - A 24/7 service providing information, advice, signposting to services and face-to-face interventions for patients, caregivers, family, and friends.\u003cbr/\u003e\u003cbr/\u003eHospice at Home \u2013 Offering a more integrated approach for patient assisting those who wish to spend their last days at home. This includes rapidly discharging patients from hospices and hospitals to avoid unnecessary admissions.\u003cbr/\u003e\u003cbr/\u003eDay Care Service \u2013 Aimed at patients who are in the last eight weeks of life and prefer to receive care at home.
