Integration, collaboration and service sustainability: Provider offers service where patient is based\u003cbr/\u003eKey criteria 4.  Improving access, reducing health inequalities and facilitating choice: Easy and equitable access to service \u003cbr/\u003eKey criteria 5.  Social value: The provider contributes positively to the local economy, social well-being of vulnerable populations, and reduced environmental impact through efficient and responsible delivery methods. \u003cbr/\u003e\u003cbr/\u003eThe assessment against each criterion was undertaken based on the current service delivery\u003cbr/\u003emodel and performance. The supplier meets all the requirements specified to the\u003cbr/\u003esatisfaction of the Commissioner, and it is expected that they will satisfy the requirements of\u003cbr/\u003ethe new contract going forwards.",
