Description

To contract multi-disciplinary support for adult Drug and Alcohol Treatment services to work with rough sleepers and those at risk of rough sleeping based in Exeter and engage them in treatment. Formation of a multi-disciplinary team which includes improving access to physical and mental healthcare. The Co-Lab will be responsible for: Recruiting a Project Co-ordinator. Notify Public Health of any challenges with recruitment and immediately that the post becomes vacant. Host the System Leadership Board and use this Board as the vehicle for overcoming any tactical or operational challenges that are unresolved. Creating the conditions for the generation of learning through the adoption of learning cycles using a Human Learning Systems approach. Ensure the wider team has a meeting room booked for team meetings utilising the funding for room hire. Host the Devon Partnership Trust posts (Band 8a Psychologist and Band 4 Assistant Psychologist).
