    "description": "The Lewisham NHS@Home Service is a healthcare delivery model designed to provide comprehensive and coordinated care for patients outside of traditional hospital settings. It leverages technology, remote monitoring, and a multidisciplinary approach to manage the health and well-being of individuals, particularly those with chronic or complex medical conditions.\nThe objective of NHS@Home is to reduce pressure on the acute healthcare system in Lewisham. This is achieved by 1) avoiding admissions and presentations to the emergency department, and 2) by facilitating supported discharges for patients already in the acute healthcare system. \nThe target population are Lewisham residents, over the age of 18, who would otherwise require hospital level intervention. It is assumed that the majority of patients accessing the service will be within the elderly, frail population.",
