The Lewisham NHS@Home Service is a healthcare delivery model designed to provide comprehensive and coordinated care for patients outside of traditional hospital settings. Lot Description: Lewisham NHS@Home Service. The current provider of the Service is meeting the contractual requirements to a satisfactory level and the intention is to award the providers the contract for the period 1st April 2025 to 31st March 2026 (with break clauses at 6 and 9 months), following PSR Direct Award Process C as there is no change to the contract terms. The lifetime value of the contract is £1,187,554, based an annual term.
